Purpose
To assure all persons in our faith community who cannot participate in the ministries, because they are home-bound or in a care facility, experience the joy and fullness of God’s presence.

Objectives

• To respond to the need for care to every one of those members and constituents who are unable to participate in our community of faith.

• To keep them connected with their community of faith on a regular basis. To alert the Pastors of any special needs.

• To coordinate our response.

Implementation

• Identify all home bound/nursing home/aclf individuals in our faith community, identifying their unique needs and unique situation.

• Pair team members with at least one of these individuals (if not paired already) listing all assignments on a master list.

• Meet together monthly - reporting on contacts, expressing special needs or significant information concerning these members.

• Have at least one in-service training every quarter.

• Encourage on-going recruitment of team members.
